If you are a new patient and wish to make an appointment for an initial visit, please call our office at the above telephone number to schedule an initial examination appointment.

The initial appointment includes:

• the necessary paper work
• diagnostic radiographs (x-rays)
• clinical examination
• consultation with the doctor
• preparation and presentation of the treatment plan in conjunction with your needs and desires

Please fill out the patient Registration Form and Medical/Dental History Form and fax or bring them in with you when you come in at your first visit. If you are unable to download and print the forms please arrive at our office approximately 10 to 15 minutes prior to your scheduled appointment to fill out the registration forms.
